Yes it is clerical. You may file the correction in the civil registrar’s office where you were born. Filing fee is P1000, though there may be other service charges. BEst to ask the office concerned directly.

Good Day, I would like to inquire on how to have my Birth Certificate Reconstructed. I was born in Singapore and file for late registration of birth in the Philippine Embassy in Singapore. The Embassy gave us a copy of the Report of Birth. Then after a few months, we went back to the Philippines for good. 5 years ago, I found out that I do not have a record of birth in NSO. NSO advised me to go to DFA and have my birth certificate reconstructed. I went to DFA last Oct 2010 to send my requirements. Every two months, I have been calling them to follow up. After 2 years, they told me they lost my papers i filed and asked me to submit a new set. I again went back to DFA to submit another set of requirements last Feb 2013. After a month I called them to follow up. They told me that the Philippine Embassy from Singapore do not have a copy of my Report of Birth. Now the DFA is requesting me to submit my original copy of Report of Birth to be forwarded to Philippine Embassy in Singapore. Since they lost the 1st set of requirements I submitted 2 years ago, I am not sure if the original document will indeed reach Philippine Embassy in Singapore. I asked them if they could provide a form or a letter stating that I have submitted an original copy. They said they cannot provide one. I asked what will happen if they will again lose my papers. They answered back and told me to process my papers to Singapore myself. I find the person in-charge rude and unprofessional.

A temporary LCR cannot accept or approve the application. THere is no other resort for this except to file in court. Please hire a lawyer to do this for you

Tanong ko lang po kung ano ang best way na gagawin ko kasi po nung kumuha ako ng NSO BC ko sa main ay still nkapangalan pa rin po sa mother ko samantalang kasal na po sila ng father ko though late registered po ako.( ipinanganak po ako na unmarried sila before) Since grade school po kasi ako surname na ng father ko gamit ko. Nung college ko lang nalaman nung kmuha ako sa nso surname ng mother ko yung nakalagay.
May nakalagay po ba sa right side ng inyong NSO birth certificate na “Legitimated through subsequent marriage”? Kasi kung oo, hindi na po talaga mapaplitan yung last name na nakalagay mismo sa inyong birth certificate dahil may nakalagay na po yun na annotation sa bandang gilid ng BC ninyo.

Mabuhay po !!! year 2000 ,,kumuha birth certificate sa NSO pero wala silang record the birth certificate ko,, so i-nad-vice kami na mag punta kung saan ako pinaganak,,nag punta ang tatay ko LCR sa Matnog Sorsogon kung saan ako pinanganak ,,chi-nek (check) nila kung may records ako at wala silang nakita ,, kaya i-nad-vice ang tatay ko na i pa-late-register ako ,,ang siste eh nakalimutan ng tatay ko ang month at day ng birthday ko pero same year ,,kaya nanghula ang tatay ko na November 11 , 1977 ,,ginamit ko yung inisyu ng LCR sa DFA ,, i was issued a passport by the DFA but it is valid for 1 year ,, for speacial purpose ,, so after a year kailangan kong i re-new yung passport ,, kumuha ako ng birth certificate sa NSO sa Q.C. at last meron na akong records, ni re-new ko ang passport ko as the usual process,i got a 5 year valid passport , then 2011 kumuha ako uli birth cert. sa NSO sa Q.C.pero ang lumabas sa birth date ko e . October 2, 1977 na,, papano ba gagawing Nov. 11, 1977,, kase lahat ng papaer ko Nov. 11 ,1977 ?

Iba po yung pangalan na ipinagamit sa akin ng nanay ko mula bata sa nakarehistro sa NSO. Sa lahat ng records ko gamit ko ang apelyido ng tatay ko, pero sa NSO Copy gamit ko ang apelyido ng nanay ko. Good Day, I would like to inquire on how to have my Birth Certificate Reconstructed. I was born in Singapore and file for late registration of birth in the Philippine Embassy in Singapore. The Embassy gave us a copy of the Report of Birth. Then after a few months, we went back to the Philippines for good. 5 years ago, I found out that I do not have a record of birth in NSO. NSO advised me to go to DFA and have my birth certificate reconstructed. I went to DFA last Oct 2010 to send my requirements. Every two months, I have been calling them to follow up. After 2 years, they told me they lost my papers i filed and asked me to submit a new set. I again went back to DFA to submit another set of requirements last Feb 2013. After a month I called them to follow up. They told me that the Philippine Embassy from Singapore do not have a copy of my Report of Birth. Now the DFA is requesting me to submit my original copy of Report of Birth to be forwarded to Philippine Embassy in Singapore. Since they lost the 1st set of requirements I submitted 2 years ago, I am not sure if the original document will indeed reach Philippine Embassy in Singapore. I asked them if they could provide a form or a letter stating that I have submitted an original copy. They said they cannot provide one. I asked what will happen if they will again lose my papers. They answered back and told me to process my papers to Singapore myself. I find the person in-charge rude and unprofessional.
I would like to know if there is a way for me to have a late registration of birth here in Manila without going through DFA? How else could I be registered in NSO?

Question lang po, makakakuha po ba ako ng passport sa DFA, sa BC from NSO wala ako midle name same with my parents, them inayos ko kung san ako naka regester, ang nalagyan lng ng midle yung sakin as per them ok lang daw un kasi ang kailangan lang naman daw sa passport is yung details ko. Usually po ang hinihingi ng DFA is a complete birth certificate from NSO. Ideally po nakalagay yung middle name nino sa NSO BC.

Very unlikely po kasi nag magkamali ang NSO dahil sina-scan lang po nila ang dokumento mula sa Local Civil Registry. Maari po kayong bumisita sa opisina namin at mag-set ng appointment para matignan ng aming nga Birth Certificate specialists ang inyong dokumento.

I went to the Civil Registry Ofc in Las Pinas to place a request for correction of her name as it appears in her death certificate (from Villahermosa to Villamossada as the correct middle name) but I was adviced to first obtain her birth certificate. i have a big prob re: cenomar i have my last cenomar 2008 BUT by feb 2013 they put a married status on my name pero mali yung first name how can i fix this since i asked the nso na lahat daw ng info ko sa bc eh pareho pero ang first name ang mali and btw the said marriage was held 2005 so i asked them why only 2013 they have this certain issues. hi verifier..its me again..i had to leave my job in qatar for a purpose of i have to file the correction for my BC’s gender error in Iligan City since I was born there only to find out that the head registrar just retired last week of july.I was there just last week and the other personnel in the office can’t accept any application since they are not authorize and advices me to wait for there city mayor to appoint new civil registrar..I mean wla ba clang someone to do the job temporarily while waiting for replacement? at ang masaklap pa dun di nila alam kelan cla mgkaroon ng new head registrar.. ganun ba tlga sistema nila? A temporary LCR cannot accept or approve the application. THere is no other resort for this except to file in court. Please hire a lawyer to do this for you

May nakalagay po ba sa right side ng inyong NSO birth certificate na “Legitimated through subsequent marriage”? Kasi kung oo, hindi na po talaga mapaplitan yung last name na nakalagay mismo sa inyong birth certificate dahil may nakalagay na po yun na annotation sa bandang gilid ng BC ninyo.

Mabuhay po !!! year 2000 ,,kumuha birth certificate sa NSO pero wala silang record the birth certificate ko,, so i-nad-vice kami na mag punta kung saan ako pinaganak,,nag punta ang tatay ko LCR sa Matnog Sorsogon kung saan ako pinanganak ,,chi-nek (check) nila kung may records ako at wala silang nakita ,, kaya i-nad-vice ang tatay ko na i pa-late-register ako ,,ang siste eh nakalimutan ng tatay ko ang month at day ng birthday ko pero same year ,,kaya nanghula ang tatay ko na November 11 , 1977 ,,ginamit ko yung inisyu ng LCR sa DFA ,, i was issued a passport by the DFA but it is valid for 1 year ,, for speacial purpose ,, so after a year kailangan kong i re-new yung passport ,, kumuha ako ng birth certificate sa NSO sa Q.C. at last meron na akong records, ni re-new ko ang passport ko as the usual process,i got a 5 year valid passport , then 2011 kumuha ako uli birth cert. sa NSO sa Q.C.pero ang lumabas sa birth date ko e . October 2, 1977 na,, papano ba gagawing Nov. 11, 1977,, kase lahat ng papaer ko Nov. 11 ,1977 ?
